Forms: Conditional Logic based on Payor Name in Database

• Have an insurance database configured
• Create a question with a Database Select 
• Have that display name be the name of the payor
• Create a branch for conditional logic 
• Select advanced logic
• Select the question that asks for insurance
• For advanced logic type in the payor name as it is in the database
• For the logic operator select "contains"
