Forms: Conditional Logic, Branching based on Calculated Age

Note: To branch based on calculated age you must set Date of Birth as either a profile field or capture it as an intake question on a public form link

• Make sure Date of Birth is being set using one of the two methods above
• Drag a branches to at least two Form Field options
• To add Logic, select Advanced Logic
• Select Calculated Age from the selector
• Select the criteria for the Conditional Logic i.e. Equal to, Greater Than, Less than
• Write in the numerical value for the criteria
