Forms: Prepopulating Form Responses

Note: Prepopulating new responses cannot be used with a public form link 

• Prepopulating form responses allows you to pull from patient fields that are set and pre-populate responses in forms
• After adding a form field, under Update Patient Profile select a patient field from the dropdown or type in (CASE Sensitive) the custom field you'd like to pull from
• Check the box "Prepopulate new responses with field" 
  • If there is no value for that profile field, the Form Field will be shown as blank

Pre-populating Database Select Responses:

• To prepopulate responses from a Database Select question type, the column name you're pulling from must have the same name as the Custom Field Name
