Public form links allow you to host a form at a link for unauthenticated endusers and users to complete. Public form links can be embedded on your company's site.
1. Allowing Public Access: Toggling this on will allow you to generate a public form link
2. Skip Intake Information: By default Public Form links require that the first page of a form to have some combination of:
- First Name + Last Name + DOB
- First Name + Last Name + Email
- Optionally: Phone Number and/or State can also be included
Enabling Skip Intake Information will you to remove the required first page and instead have a "getting started" button that a user must click to start the form. IMPORTANT: If you're using Skip Intake any time a response is made a new patient will be created because we are unable to match against existing contacts without the Intake Question Type.
3. Public Identifier: Allows you to create different URL strings for public forms to track performances for things such as different Landing Pages. Responses on the timeline will show the Public identifier for the form. Note: Every time you generate a public form link you must specify the Public Identifier (if you choose to use one)

3A. Contact Type: If a public form submission should generate a contact type OTHER THAN the default patient contact type, you must specify that when generating a public form link. Note: Every time you generate a public form link you must specify the Contact Type (if you choose to use one other than patient)
4. Next Form: After Completing the form, you can select another form that a patient is prompted to fill out without requiring them to leave the page.
5. Required Public Form Link Information: By default Public Form links require that the first page of a form to have some combination of:
- First Name + Last Name + DOB
- First Name + Last Name + Email
- Optionally: Any combination of Phone Number, State, and Gender can also be included as Optional OR Required fields for a public form link.
6. Redirect to Other Form on Page Load: If an old form is outdated and no longer in use, you can specify another form that should load in its place.
Public Form Settings
- If Show Language is enabled it allows a patient to select whether the first page of the public form should be in English or Spanish (As shown below)
- If download submission is enabled, after completing the form, the person who filled it out will have the option to download their responses on the Thank You Page. Responses will be exported and Saved as a PDF.
