Notes and forms have a lot of similarities.
They are the same in the sense that both can be templated and the builder is the same (you can have the exact same type of questions in both).
Our vision for the difference between them is that one is Patient-Facing (a form), and one is Internally-Facing (a note).
- The form for example allows for customizations of what the patients see when they're sent the form.
Specifying a Type
- Selecting Patient Facing will create a Patient-Facing form
- Selecting Internal Note will create an Internal Note that your team can take
- The form also allows for it to be sent directly out through a public link (if you wanted to put it on your website for example)
Note vs Form on Timeline
On the timeline:
- Notes are Grey and have Notes written on the top righthand corner of the box
- Forms are Navy and have Forms written on the top righthand corner of the box
Taking a Note
- From a Patient's Profile Click Compose
- Select Note from the Tab
- If you would like to take a free-from note Add a Title for the note and type in the note
- If you would like to use a Structured Note select the template for the Structured Note that you would like to use
- (To complete a patient-facing form on behalf of a patient repeat the above steps but click on the Form tab instead of Note)
