Hide Logo: Hides the logo at the top of the form

Ask Public Form One Question at a Time: Shows the required fields for a public form which are a combination of (First Name, Last Name, Phone Number, Email, DOB) on individual pages for a form

Hide Progress Bar: Hides the progress bar that shows the completion percentage at the bottom of the form.

Hide background (hides background card and background color): Makes the background for a form transparent.

Public Form Prefix: Lets you add words before the required fields for additional clarity. I.e. (Patient) to clarify Patient First Name should be added.

Public Form Description: Adds text above the first field (question) for a public form.

Public Form Bottom Description: Adds text below the last field (question) for a public form.

Show restart link on thank you page (public form link only): After completing the form using a public form link the user is prompted to fill out another blank version of the form again.
