Patient Portal: Viewing / Submitting Forms and Files

In the portal, patients can view previously submitted forms, and files they or you have uploaded, and submit any forms that are still resubmitted.

• Note: Even if you have texted or emailed a form link to a patient to complete, if it is unsubmitted there will still be a notification in the portal that the file is incomplete
