Patient Profile: Selecting and Submitting Internal Forms

Internal Forms allow your care team to record structured notes or fill out a form on behalf of a patient.

Selecting a Form:

• Click on the Patient Profile Page
• Click Compose
• Select Form
• Select the Form that you want to fill out from the dropdown
• Click Next

Submitting a Form:

• After selecting the form and filling it out click Submit Response
